egr error code kicks off the check engine light

Q.Q1. 1996 accord 4 cyl vtec - egr error code kicks off the check engine light, but EGR valve appears to function fine - when vac is applied it opens the recirulcation port.
Could there be another problem causing this besided the egr valve? Q2: there is a vac hose that comes from the rear motor mount and goes up to a solenoid valve mounted on a rubber isolater mounteed on the firewall just above the mount.
There are 2 ports on that valve. One is hooked up and the other one has no vac hose on it. Is this normal? I am asking because last year I replaced the rear mount, but still get l a lot of vibration at idle.

More Info required when the vacuum is applied does the engine stall ?
the second for the mount, one port will have vacuum and the other will not until its commanded to come on and then the solenoid will open up and allow vacuum to flow.

Addition 1. No the engine does not stall when the hose is hooked up.
2. Here is the valve - is there supposed to be a vac hose on the open port?

Answer yes vacuum on 1 port .
ok the engine does not stall the egr has now flow and the egr ports are plugged up and need to be cleaned. Order a new gasket set and use a pick to clean out the carbon in the holes.

Addition 1. I already took off the egr and when I apply vaccum to the port it works. What am I supposed to clean out? It looked like there was no carbon blocking anything. I cleaned it up a little with a screw driver here and there.
2. So you are saying the hose is correct on the one port and the other port has no hose on it as shown in my picture?

More Info required you removed the egr plate ?
applying vacuum to the egr is telling me the egr is holding vacuum.
if you look on the hood sticker it will show the correct vacuum hose routing.
the second port needs to have a vacuum line one that goes to the rear mount and the other to the manifold.

Addition I only removed the EGR valve itself.
Can you supply a diagram for the vac hoses I need to get on there? I dont see the valve we are talking about included on the under hood diagram.

Answer ok this is not where the egr ports are located that are plugged.
if you remove the plate i mentioned you will see what im talking about.

Is the plate you are talking about right below the part labeled "EGR CHAMBER"?

Answer yes this is correct. Remove the bolts holding the plate and directly in the middle of each hole use a pick and pock it through the holes and you will feel it poke through.
